Parent Referral Form – Orange

This form is for parents to refer a child for support from Vista Support Services. 

Please follow the steps below to ensure the referral is processed correctly:

  1. Complete the form, ensuring all required fields are filled in and any additional relevant information is included.

  2. Please ensure the consent section is completed.

  3. Submit the form.

  4. Vista Support Services CIC will be in touch once theform is reviewed.

Terms & Conditions

All children and young adults must acknowledge Vista’s Student Support Code of Conduct. VISTA staff reserve the right to withdraw from the service/ end the session should inappropriate behaviour be displayed.

Full payment is required in advance.

To cancel or reschedule your session please contact your support worker directly this can be done via their email/ mobile phone or call 01727 519128. Failure to give 24 hours’ notice will result in your session still being charged.

Consent

Consent and Information sharing – Parent/Guardian consent is required to enable information sharing with the identified service. Young people should be aware of the request for a Support Service and be asked for their consent.
