Please follow the steps below to ensure the referral is processed correctly:
Complete the form, ensuring all required fields are filled in and any additional relevant information is included.
Do not complete the consent section—this will be completed by the parent or guardian.
Submit the form.
An email will automatically be sent to the parent or guardian with a secure link.
The parent or guardian will review the information entered (read-only) and complete the consent section.
Once the parent has signed and submitted, the completed referral will be securely received by Vista Support Services.
