This form is for schools to refer a pupil or family for support from Vista Support Services. The referral will be reviewed once parental or guardian consent has been provided.

Once submitted:
An email will be sent automatically to the parent or guardian with a secure link.
They will be able to review the referral details (read-only) and complete the consent section.
After submission, use the “Add Another Child” button to refer the next child.
